Required Student Forms

Saint Andrew School complies with the State of Illinois health requirements for physicals, immunizations, eye examination and dental exams. These requirements must be completed on specific State issued forms.

Certificate of Child Health Exams are required for new students entering Early Childhood Education (ECE), all Kindergarten students, all 6th Grade students and any new students in grades Early Childhood Education - 8th.

Eye exams are required for children entering Kindergarten and any student in 1st - 8th Grade enrolling for the first time in a school in the State of Illinois.

Dental Exams are required for students entering Kindergarten, 2nd Grade, 6th Grade and any new student in grades K-8th.

Please complete all athletic forms prior to the start of each individual sports season

Saint Andrew School complies with the Illinois State Board of Education and the Archdiocese of Chicago Office of Catholic Schools medication procedures. No school personnel shall administer any medication (prescription or non-prescription) unless the school has the student's current and complete Medical Authorization Forms on file. Saint Andrew School complies with State of Illinois requirements and the Archdiocese of Chicago Office of Catholic Schools Asthma Guidelines for the management of asthma within the school setting.
